In the Diocese of Lafayette in Indiana, introducing a Marriage Case begins at the parish level. A person (also known as the petitioner) should work with the parish to complete an application. Parishes often have paper applications. The same application is available here. A petitioner may work with a parish to complete a paper version or this PDF version of the application form.
In addition, the case history questionnaire is also available (formal cases only).
The application can be filled out in PDF format. The case history questionnaire is in a Microsoft Word format. Both may be downloaded to your personal computer. Both the completed application and case history questionnaire must be
printed out and signed in order to submit them by mail to the Tribunal at 2300 S. 9th St., Lafayette, IN 47909-2400. In order to protect your privacy, electronic submission of documents is not available at this time.
Please note, the case history questionnaire is used only in formal cases. After the Tribunal receives an application, trained staff determine which kinds of cases are possible. If it is determined the case should be processed as a formal case, then the petitioner is sent a paper copy of the case history questionnaire and directions for completing it. For convenience, the case history questionnaire is available here, but it should only be completed and mailed to the Tribunal if and after the Tribunal requests it.
